Anatomical, physiological and functional neuroimaging studies suggest that the cerebellum also participates in the organization of higher cognitive function. A 64-year-old man developed vertigo, nausea, and vomiting due to bilateral cerebellar infarction. Ten days after treatment with antiplatelet agent, the patient showed improvement of vertigo but revisited our hospital due to memory impairment, comprehension deficit, repetition failure, alexia, agraphia, acalculia, visuospatial dysfunction and frontal lobe dysfunction. Clinical onset, neuroimaing findings, and neuropsychological testings led us to the diagnosis of strategic infarct dementia after cerebellar infarction
